Skin Burns

The severity of a burn is related with its deepness more than with the agent that caused it. They are classified into 3 and even 4 degrees that compromise the skin layers, muscle and bones.

First Degree Burn

They are limited to the epidermis. They are characterized by heat, pain, moistening and reddening of the burned surface, but rarely show blistering or charring of tissue. They often heal in 3-7 days and rarely leave a scar, and often include sunburn and minor scalds.

Second Degree Burn

They are characterized as "superficial" or "deep." They penetrate deeper than a first degree burn and destroy the epidermal layers, extending into the dermis layer. Sweat glands and hair follicles can be damaged. They are terribly painful, producing blisters, severe pain, and redness. The skin can appear blotchy white to cherry red.

Most superficial second degree burns heal from 10-21 days, but change skin color and pigmentation. They can be ivory or pearly white colored and may require a process known as skin grafting treatment.

Most skin grafts replace burned skin, while others temporarily cover and protect the skin as it heals on its own. In this procedure, a piece of healthy skin is taken from the person's body (autograft), from another living or dead person (allograft), or from another species (xenograft)—usually pigs. The skin graft is surgically sewn over the burned area after removing all dead tissue and cleaning the wound.

Third Degree Burn

They destroy all the epidermal and dermal skin layers. The damage extends below the hair follicles and sweat glands to subcutaneous (fat) tissue. The skin becomes charred and leathery and often appears depressed compared to the surrounding tissue. The skin gets bright red, waxy white, tan or brown; there are no blisters. These burns may cause massive swelling. They are usually not painful because the injury has destroyed nerve endings.

Burn Classification as an Injury

The size of the body's burned area also helps to classify a burn as minor or major. Doctors have several definitions to categorize burns and they may not correspond to a person's understanding of the terms.

Doctors, to determine the severity of a burn, estimate the percentage of the body's burned surface. For adults, doctors use the rule of nines, which divides almost all of the body into sections of 9% or of 2 times 9% (18%). For children, doctors adjust these percentages according to their age. Different body areas grow at different rates.

A fluid leaks into the burned tissue from the blood vessels, causing swelling and pain. In addition, damaged skin and other body surfaces are easily infected because they are unable to act as a barrier against invading organisms.
